Understanding Idiopathic Hypersomnia (IH) and Its Impact Idiopathic Hypersomnia (IH) is a chronic neurological sleep disorder characterized by excessive daytime sleepiness (EDS). The exact cause of IH is unknown, hence the term 'idiopathic,' meaning of unknown cause. While the exact prevalence is not well-established, it is considered a rare disorder. The Role of Medication in IH Management Medical professionals often prescribe medications to help manage the symptoms of IH. While the Food and Drug Administration (FDA) approved the first medication specifically for IH in 2021 – calcium, magnesium, potassium, and sodium oxybates (Xywav) – many other medications are used off-label. Off-label use means a drug is prescribed for a condition it hasn't been officially approved for, but for which it may still be beneficial. These medications generally fall into a few categories: Stimulant medications: These are used to increase alertness and combat the overwhelming sleepiness. Non-stimulant medications: These work by modifying brain chemicals to promote wakefulness. Oxybates (sodium and lower sodium): Medications like Xywav and its predecessor, sodium oxybate (Xyrem, Lumryz), aim to promote deeper night-time sleep, which in turn can reduce daytime fatigue. It's important to note that Xyrem is high in sodium, which can be a concern for individuals with IH who may have an increased risk of heart disease. Xywav offers a lower sodium alternative with the same active ingredient. Despite these medical interventions, research indicates that a significant percentage of individuals with IH (39–64%) find that off-label medications alone are not sufficient to manage their symptoms effectively. This often leads patients to explore complementary strategies. Exploring Non-Pharmacological Approaches: The Potential of Exercise Given the limitations of medication alone, many individuals with IH seek alternative or complementary strategies to manage their condition. A survey of 129 people with IH revealed that a remarkable 96% were actively trying approaches beyond medication. Among these, exercise emerged as a popular choice, with 58% incorporating physical activity into their management plan. While the idea of exercising when experiencing profound fatigue might seem counterintuitive, emerging research and expert opinions suggest that physical activity could play a beneficial role in managing excessive daytime sleepiness associated with IH. Evidence Supporting Exercise for Sleep Disorders Several studies, though some are small, offer promising insights into the connection between exercise and improved sleep quality and reduced daytime sleepiness: A 2018 study involving older adults with sleep disorders found that those who engaged in a daily exercise routine reported less daytime sleepiness compared to their sedentary counterparts. Similarly, a small 2019 study indicated that individuals who increased their daily step count experienced a reduction in daytime sleepiness. These findings suggest that regular physical activity, even in moderate amounts, may help to mitigate the overwhelming sleepiness experienced by individuals with sleep disorders, including IH. How Exercise Might Help with IH Symptoms Nick Pellizzari, a registered kinesiologist, explains that exercise can contribute to better sleep through its effects on hormone levels. He notes: “Exercise can improve sleep by increasing melatonin levels in the body. Melatonin is a hormone involved in regulating sleep-wake cycles.” Furthermore, Pellizzari suggests that exercise may lead to a decrease in ghrelin levels. Ghrelin, often referred to as the “hunger hormone,” has been found to be elevated in individuals experiencing excessive daytime sleepiness. By potentially lowering ghrelin, exercise might indirectly help manage some aspects of IH. Types of Exercise to Consider For individuals with IH looking to incorporate exercise, the key is to start gradually and find activities that are sustainable and enjoyable. The focus should be on consistency rather than intensity, especially in the beginning. Aerobic exercise: Activities that elevate your heart rate, such as brisk walking, cycling, swimming, or dancing, can improve cardiovascular health and overall energy levels. Strength training: Incorporating exercises that build muscle strength can improve body composition and metabolism. Flexibility and balance exercises: Activities like yoga or tai chi can promote relaxation and improve body awareness. Getting Started and Staying Consistent For those who find the prospect of exercise daunting due to their IH symptoms, Pellizzari offers practical advice: “If this seems unattainable, remember that something is better than nothing. Try starting with a 5- to 10-minute walk and building this up over time to longer durations.” The emphasis is on making small, achievable steps. Consistency is more important than the duration or intensity of the exercise, especially when first starting. Gradually increasing the duration and frequency of exercise as tolerated can help build momentum and make it a sustainable part of a management plan. When to Consult a Doctor While exercise can be a valuable tool in managing IH, it is crucial to consult with a healthcare professional before starting any new exercise program, especially if you have a chronic condition like Idiopathic Hypersomnia. Your doctor can help you: Determine the safest and most appropriate types and intensity of exercise for your individual condition. Monitor your response to exercise and adjust the plan as needed.
