Thyroid Eye Disease (TED), also known as Graves’ ophthalmopathy or Graves’ orbitopathy, is a condition that affects the eyes and can sometimes accompany thyroid disorders, most commonly Graves’ disease. It's an autoimmune condition where your body's immune system mistakenly attacks the tissues around your eyes. This can lead to a range of uncomfortable and sometimes distressing symptoms. About 90% of TED cases are linked to Graves’ disease, an autoimmune disorder that causes the thyroid gland to produce too much thyroid hormone (hyperthyroidism). However, it's important to know that TED can sometimes occur even if your thyroid hormone levels are normal, low, or if you have other thyroid conditions like Hashimoto's thyroiditis.
The development of TED is not fully understood, but certain factors increase your risk. These include being female, particularly between the ages of 40-44 or 60-64, and males between 45-49 or 65-69. Smoking is a significant risk factor and can worsen TED symptoms. If you have other autoimmune conditions like lupus, celiac disease, or rheumatoid arthritis, your risk might also be higher. Certain ethnicities, like African Americans, followed by white and Asian individuals, may have a higher susceptibility. Additionally, if you have undergone radioactive iodine therapy for thyroid cancer, this can be a trigger.
The initial signs of TED can be subtle, and for some, these eye changes might be the very first indication of an underlying thyroid problem. Pay close attention to any changes in your eyes. Common symptoms include:
These symptoms can develop gradually or appear more suddenly. They might affect one or both eyes. It's crucial to document any changes you observe, as this information will be helpful for your doctor.
For many, the eye symptoms of TED can appear within 18 months of being diagnosed with Graves' disease. However, this isn't a strict rule. Sometimes, TED symptoms can precede a formal diagnosis of a thyroid condition, acting as an early warning sign. In other instances, TED might develop even when thyroid hormone levels are within the normal range, or even if you have hypothyroidism, though it's more strongly associated with hyperthyroidism.
