Asthma is just as common in adults over 65 as it is in children β but far more dangerous. Older adults with asthma are more than five times more likely to die from the condition than younger patients. The disease is frequently misdiagnosed or dismissed as a normal part of aging, and it overlaps with

Asthma is not just a childhood condition. Around 7.2% of older adults in the United States have asthma β but asthma is riskier for older adults than younger people because they face special challenges. Despite how common it is, the condition is routinely missed, underestimated, and undertreated in this age group. nih
The consequences are serious. During the period from 2001 through 2021, the death rate attributable to asthma was 27.1 per million for patients aged 65 years or older, compared with 5.4 per million for patients aged 18 to 34 years. That's a fivefold difference β driven largely by delayed diagnosis, overlapping conditions, and treatment gaps. nih
Aging changes the lungs, the immune system, and the way symptoms appear. When compared to younger patients, elderly people with asthma have different drug responses, higher rates of bronchial hyperreactivity, a more severe disease phenotype, and a lower prevalence of atopy. Atopy refers to the inherited tendency to develop allergic conditions β its relative absence in older patients means asthma in this group often doesn't look the way most people expect. nih
Asthma in older adults is shown to have a significant impact on quality of life. Many times, asthma in the elderly coexists with conditions such as obesity, decreased immunity, and chronic obstructive pulmonary disease (COPD) β all of which are common among this population. As a result, asthma can often be complex and difficult to spot. PubMed
There are also two distinct groups within this population. At least two phenotypes exist among elderly patients with asthma: those with longstanding asthma have more severe airflow limitation and less complete reversibility than those with late-onset asthma. In other words, someone diagnosed at age 70 may have a quite different disease profile from someone who has managed asthma since childhood. Australian Prescriber
Asthma in older adults is one of medicine's more persistent blind spots. Asthma in the elderly remains underrecognized, undertreated, and a challenge to properly diagnose and treat. PubMed
Part of the problem is symptom overlap. Wheezing is a symptom of asthma and a number of other conditions, including COPD. Clinicians must keep in mind that physical exams in older adults with asthma may sometimes misguide the diagnosis. Breathlessness can also be attributed to heart failure, deconditioning, or anxiety β especially in patients who have multiple conditions simultaneously. PubMed
Patients themselves often contribute to the delay. Older patients may fail to recognize respiratory symptoms as abnormal β attributing shortness of breath to simply getting older, rather than to a treatable condition. This normalization of symptoms is a significant barrier to timely care. PubMed Central
Lung function testing adds further complexity. Considering that the ratio of FEV1 over forced vital capacity β a standard measure of lung function β decreases with age, clinicians must use age-adjusted values when diagnosing asthma. Clinicians may also face challenges with spirometry for those who are frail and who have poor cognition and coordination. Spirometry is the gold standard breathing test for asthma, but it requires sustained effort that some older patients cannot easily provide. PubMed
[REVIEWER: add clinical insight here β e.g., how you approach the diagnostic workup when symptoms overlap with COPD or heart failure in an older patient, and what additional testing you find most useful]
One of the most clinically difficult situations in respiratory medicine is when asthma and COPD exist together β known as asthma-COPD overlap (ACO).
COPD is chiefly a disease of older people, so the prevalence of asthma-COPD overlap increases with age. Age-related physiological changes may contribute to airflow limitation. Comorbidities are frequent in older people and present challenges for management. nih
A common clinical scenario is an older former smoker with partially reversible or fixed airflow obstruction and evidence of atopy, demonstrating "overlap" features of asthma and COPD. Asthma-COPD overlap syndrome becomes more prevalent with advancing age as patients respond less favorably to guideline-recommended drug therapy.
This overlap matters for treatment: the wrong diagnosis can mean the wrong medication, which in turn can mean avoidable hospitalizations and worsening disease.
The numbers are stark. Elderly patients have the highest reported asthma-related mortality at 51.3 per million persons. Older asthmatic patients above age 55 had a fivefold higher risk of overall mortality than younger adults in a large retrospective cohort study of emergency department visits. Annals of Allergy, Asthma & Immunology
By 2030, it is projected that there will be more than 5 million persons with asthma above the age of 65. As populations age globally, the scale of this underrecognized problem will only grow. Annals of Allergy, Asthma & Immunology
Older adults aged 60 and over represent a critically understudied subgroup in asthma research. Atypical symptom presentation, high rates of misdiagnosis, and systematic exclusion from clinical trials have contributed to a paucity of evidence guiding geriatric asthma management. Exclusion from trials is a structural problem that leaves clinicians with limited evidence specifically tailored to older patients. ScienceDirect
The goals of asthma treatment in older adults are the same as in younger patients: control symptoms, prevent attacks, and preserve lung function. But the path there is more complicated.
Cognitive impairment can affect self-management skills. Older people are less likely to use inhalers effectively. Dexterity may be affected by osteoarthritis and should be considered when choosing an inhaler device. A patient who cannot physically actuate a pressurized metered-dose inhaler correctly will not receive the intended dose β regardless of what's prescribed. nih
Device choice matters significantly. Dry powder inhalers, soft mist inhalers, and spacer devices each have different coordination requirements, and matching the device to the individual patient's physical and cognitive capacity is a core part of managing asthma in older adults well.
A personalized self-management program for older patients with asthma β targeting barriers to self-care such as poor inhaler technique, limited understanding of the role of medicines, and environmental triggers β was shown to reduce exacerbations and improve quality of life. nih
Medication interactions also require careful attention. Aging can affect the metabolism of drugs because of age-related decline in renal and hepatic metabolism. Older patients are frequently on multiple medications for multiple conditions, and some drugs β including certain beta-blockers used for heart disease and NSAIDs used for pain β can worsen asthma symptoms. Annals of Allergy, Asthma & Immunology
Vaccination is another key element. Since infections are one of the leading causes of exacerbation of asthma and COPD, getting the appropriate vaccines will help to reduce those exacerbations β including the yearly influenza vaccine and pneumonia vaccines.
[REVIEWER: add clinical insight here β e.g., your approach to reviewing inhaler technique in older patients during consultations, or how you balance asthma medications against cardiac or renal considerations]
Is asthma common in elderly people?
Yes. Around 7.2% of adults aged 65 and older in the United States currently have asthma β a prevalence similar to that seen in children. Despite this, it remains significantly underdiagnosed and undertreated in older populations, partly because symptoms are often attributed to aging or other conditions.
Why is asthma more dangerous in older adults?
Older adults with asthma are over five times more likely to die from the condition than younger patients. Age-related changes in lung function, a higher burden of other health conditions, delayed diagnosis, and barriers to effective inhaler use all contribute to worse outcomes in this age group.
Can asthma start for the first time in old age?
Yes. Late-onset asthma β diagnosed for the first time in someone over 65 β is well-recognized. It often has a different profile than childhood-onset asthma, with less allergic involvement and more overlap with other respiratory and cardiac conditions.
How is asthma different from COPD in elderly patients?
Both cause breathlessness, wheezing, and airflow obstruction, which makes them hard to distinguish. A key difference is that asthma's airflow obstruction is typically reversible (it opens up with treatment), while COPD's is largely fixed. In older adults, both conditions frequently coexist β called asthma-COPD overlap β requiring careful assessment to manage correctly.
What should older adults with asthma watch for?
Worsening breathlessness, more frequent use of a rescue inhaler, nighttime waking with cough or wheeze, and reduced exercise tolerance are all warning signs that asthma is not well controlled. Any of these changes warrants a prompt review with a doctor β not an assumption that symptoms are just part of getting older.
Practical Takeaway
Breathlessness and wheeze in an older adult should never be dismissed as inevitable aging. Asthma at 70 is treatable β and well-controlled asthma dramatically reduces the risk of hospitalization and death. If you or someone you care for has unexplained respiratory symptoms, push for a proper assessment that includes lung function testing interpreted with age-appropriate values, a review of all current medications, and hands-on inhaler technique checks. The gap between how asthma is managed in older adults and how it should be managed is large β and closeable.
