ESIC hospitals and dispensaries in Bihar explained β coverage, referral hospitals, cashless treatment, and where the gaps still are for insured workers.

Most ESIC cardholders in Bihar have never actually visited their nearest dispensary. That's not laziness. It's often because nobody explained where it is, what it covers, or what happens if the nearest one is 40 kilometres away.
The Employees' State Insurance scheme covers workers in the organised sector below a set wage threshold, along with their families, funded through small monthly contributions from both employee and employer. On paper, it's a solid safety net. In Bihar, whether that safety net actually reaches you depends a lot on geography.
ESIC health access in Bihar runs on three levels, and understanding the order matters more than most people realise. First comes the dispensary, usually combined with a branch office and referred to as a DCBO. This is where routine consultations and basic treatment happen.
Above the dispensary sits the ESI hospital, for inpatient care and specialities the dispensary can't handle. And filling the gaps where ESIC has no hospital of its own, the corporation empanels private hospitals to provide cashless treatment for specific conditions.
According to the most recent figures placed before Parliament, Bihar has four ESI hospitals: two in Patna district, one in Munger, and one in Rohtas. There are 17 ESI dispensaries across the state. Put those numbers next to Bihar's 38 districts and you'll notice the gap immediately β the same government reply flagged that 26 districts had no ESI hospital or dispensary at all at that time.
If there's one facility worth knowing in detail, it's the ESIC Medical College and Hospital at Bihta, just outside Patna. Established in 2021 and affiliated with Bihar University of Health Sciences, it's grown into one of the larger hospitals in Patna district and also trains MBBS students.
Here's the part that surprises people: this hospital doesn't only treat ESIC-insured patients. Government data shows it treated well over a million non-insured persons through OPD services in the years since May 2022. In other words, Bihta functions almost like a public hospital for the surrounding area, regardless of whether someone holds an ESIC card. That's worth knowing if you live nearby and aren't covered under the scheme yourself.
A 100-bed ESIC hospital for Muzaffarpur received in-principle approval a while back, aimed at improving access in North Bihar. Whether it's fully functional by now is genuinely worth confirming with the regional ESIC office directly, since project approvals in India don't always move at the pace anyone would like.
This is where confusion causes the most trouble. Empanelled hospitals around Patna and Muzaffarpur cover specialities like cardiology, neurology, gastroenterology, nephrology and dialysis, orthopaedics, ophthalmology, general and laparoscopic surgery, obstetrics and gynaecology, and oncology.
But cashless treatment at these hospitals generally requires a referral from your nearest ESI dispensary or Insurance Medical Officer first. Walk in without one for a non-emergency issue, and you may end up paying out of pocket, then filing for reimbursement later, which takes time nobody wants to spend while unwell.
In clinical practice, this referral step is exactly where patients lose the most time. They assume the ESIC card alone gets them cashless treatment anywhere on the empanelled list, when the scheme is actually built around a documented referral chain. Knowing this before an appointment saves a genuinely stressful scramble later.
Emergencies are the exception. Genuine emergencies can be treated at the nearest hospital, insured or empanelled or not, with reimbursement claimed afterward. Still, it helps to already know your nearest hospital and ambulance service before you're in a crisis, rather than searching for one mid-emergency.
If you live in one of the districts without a nearby ESI facility, waiting for the network to expand isn't a realistic plan. It helps to know your alternatives for routine care. You can search general physicians in Patna, or look up dermatologists and gastroenterologists in Patna independently and compare consultation costs against the time cost of travelling to a dispensary.
For family members not covered under the ESIC card themselves, gynaecologists, paediatricians, and dentists can be searched by city rather than waiting on a dispensary referral that may not even apply to them. Workers dealing with joint strain or repetitive-work injuries often need orthopaedic specialists or general surgeons faster than the referral chain allows, especially outside Patna.
It's also worth browsing ophthalmologists in Patna and dieticians in Patna, since eye strain and diet-linked conditions are common among factory and shift workers and don't always need a full hospital referral to address early. For anyone managing a chronic condition alongside work, checking endocrinologists, cardiologists, and pulmonologists near you is a reasonable starting point.
