West Bengal's government health insurance system just got more complicated β and more useful, if you know how to navigate it.
The state has always run its own health coverage independent of the Centre. That independence came at a cost: West Bengal families couldn't use their scheme cards at hospitals outside the state. Mid-2026 changed that. The restructuring that brought PM-JAY integration alongside the brand-new Mukhyamantri Swasthya Bima Yojana means the system is now layered in a way that most families haven't had time to understand yet.
Let's fix that.
Launched on 30 December 2016, Swasthya Sathi provides basic health cover for secondary and tertiary care of up to βΉ5 lakh per family per year β cashless, paperless, and smart card-based. All pre-existing diseases are covered, and there is no cap on family size. Parents from both sides of the couple are included. So are dependent family members with disabilities.
The entire cost is borne by the state government β there is no premium or contribution required from the beneficiary. That's the headline feature most people know. What fewer know is that the smart card is issued in the name of the senior-most female member of the family β and if that member's name or Aadhaar details are mismatched with hospital records, the cashless process breaks down at the counter. It happens more than it should.
Over 2.5 crore families in West Bengal are enrolled, with more than 95 lakh hospitalisations approved to date. Those are real numbers β the scheme has demonstrably reduced catastrophic health spending for poor and middle-income households.
In mid-2026, West Bengal's public health insurance system underwent a significant transformation. Swasthya Sathi transitioned to become the foundational platform for a new multi-layered model, integrating eligible beneficiaries into the national Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY).
Why does PM-JAY matter? Because it's portable. A general physician in Kolkata won't help you if you fall ill in Delhi. PM-JAY-empanelled hospitals exist across India, so for families with members who work or study outside West Bengal, the integration is genuinely useful.
But not everyone qualifies under PM-JAY's central criteria. The Mukhyamantri Swasthya Bima Yojana (MMSBY) was officially launched on 16 August 2026 to protect eligible Swasthya Sathi holders who fall outside AB PM-JAY. MMSBY provides cashless coverage of βΉ5 lakh per family per year for this group.
So the current architecture, plainly stated: Swasthya Sathi is the base for everyone. PM-JAY applies to those meeting central eligibility. MMSBY covers the gap in between.
MMSBY excludes families with annual income above βΉ8 lakh, as well as employees or pensioners under the West Bengal Health Scheme (WBHS), CGHS, ESI, or any Group Medical Insurance operated by government-controlled bodies. Family members above 70 years of age are also excluded from MMSBY
That last point deserves attention. Elderly parents β exactly the family members most likely to need hospitalisation β may not be covered under MMSBY. They may, however, qualify under a separate PM-JAY provision: since 2024, all senior citizens aged 70 and above are eligible for PM-JAY regardless of income, receiving a dedicated Ayushman Vay Vandana Card for cashless hospitalization of up to βΉ5 lakh per year. Check that separately from your Swasthya Sathi card.
If you're a state government employee, your coverage is under the West Bengal Health Scheme (WBHS) 2008 β not Swasthya Sathi. Private sector workers earning below βΉ21,000 a month typically fall under ESIC. Everyone else β self-employed, informal workers, homemakers β is the primary audience for Swasthya Sathi and MMSBY.
Coverage under Swasthya Sathi includes secondary and tertiary hospitalisation, consultations during admission, diagnostics, medicines, hospital food, and post-discharge follow-ups. Here's the gap that catches families flat-footed: outpatient care isn't covered. Swasthya Sathi covers only inpatient treatment at empanelled hospitals β OPD visits and outpatient medicines are not covered.
That means a visit to a cardiologist in Kolkata for a routine ECG, a session with an endocrinologist in Kolkata to manage thyroid medication, or a consultation with a dermatologist in Kolkata for a skin condition β all out of pocket, unless you're admitted. Families managing chronic conditions like diabetes, hypertension, or asthma carry this ongoing cost completely on their own. No government scheme currently addresses it well.
This is the single biggest practical limitation of West Bengal's scheme structure, and in my reading of how the system actually works, it's the gap most families don't discover until they're at the OPD counter being turned away.
If a procedure is covered under Swasthya Sathi, you'll need a specialist at an empanelled hospital. That's where advance planning matters.
Families in Kolkata can verify which doctors practice at empanelled hospitals through DOCTAR β checking a general surgeon in Kolkata, orthopedist in Kolkata, or gynecologist in Kolkata before a procedure, rather than during a crisis. For pediatric care, pediatricians in Kolkata and pediatric surgeons in Kolkata are listed with hospital affiliations visible on each profile.
Neurological conditions are a common reason families need specialist referrals β neurologists in Kolkata and neurosurgeons in Kolkata at empanelled hospitals would qualify for covered procedures. Similarly, oncologists in Kolkata at covered facilities can treat cancer cases under the scheme.
For everyday specialist needs that fall outside hospitalisation β a gastroenterologist in Kolkata for digestive issues, a pulmonologist in Kolkata for breathing conditions, a nephrologist in Kolkata for kidney monitoring, a urologist in Kolkata for urinary concerns, or a psychiatrist in Kolkata for mental health β transparent consultation fees are listed upfront on DOCTAR so families can plan the out-of-pocket expense.
Post-surgical recovery often involves physiotherapists in Kolkata for rehabilitation, or home visit doctors in Kolkata for follow-up care β also primarily out-of-pocket costs not captured by scheme coverage.
For children's eye health, dental issues, or hearing problems, ophthalmologists in Kolkata, dentists in Kolkata, and ENT specialists in Kolkata are all searchable and bookable with no platform fee.
Unlike Swasthya Sathi, WBHS does cover outpatient treatment through reimbursement. For treatment at non-empanelled hospitals, the reimbursement is the actual amount paid or 60% of the approved cost β whichever is lower. Keep every receipt. Reimbursement claims rejected for missing documentation are one of the most avoidable problems government employees face.
WBHS beneficiaries consulting an endocrinologist in Kolkata or cardiologist in Kolkata at a non-empanelled private hospital should confirm the reimbursement rate for that specific specialty before booking β rates vary by procedure type.
West Bengal has over 2,000 empanelled hospitals, including top private facilities and all government hospitals. Families can visit and filter by district to find approved hospitals near them.
