Getting through NEET PG is only half the job β the counselling round in Bihar decides which college and branch you actually land in.

Clearing NEET PG feels like the finish line. It isn't. The counselling process that follows decides whether your rank actually turns into a seat in the branch you wanted, or a compromise you'll live with for three years.
Bihar runs its own state counselling alongside the All India Quota (AIQ) round conducted by the Medical Counselling Committee (MCC). Understanding how these two tracks interact β and where they don't β is where most candidates lose time they didn't need to lose.
Bihar's postgraduate medical seats are split into two broad pools. Fifty percent go into the All India Quota, filled through MCC's centralised process. The rest stay with the state, filled by the Bihar Combined Entrance Competitive Examination Board (BCECEB) through state counselling.
If you're a Bihar domicile candidate, you're eligible for both. Non-domicile candidates can usually only compete for the AIQ share and any open seats in private or deemed colleges. This distinction alone changes how many real choices you have on counselling day, so check your domicile status early rather than assuming.
Government medical colleges in the state β including the ones in Patna, Darbhanga, and Bhagalpur β fill up fast in the first round. Private and deemed institutions tend to have vacancies stretch into later rounds, sometimes into mop-up and stray vacancy rounds.
Every counselling cycle, someone gets stuck at the verification desk over a missing document, and it's rarely something exotic. It's usually a NEET PG scorecard printout, an internship completion certificate, or a caste/category certificate that wasn't attested correctly.
Keep these ready well before your allotted slot: NEET PG admit card and scorecard, MBBS degree or provisional certificate, internship completion certificate, permanent registration with the State Medical Council or NMC, domicile certificate, category certificate (if applicable), and a valid photo ID. Carry originals plus two photocopy sets β offices rarely have a photocopier free when you need one.
The AIQ round is uniform nationwide β same portal, same rules, regardless of state. State quota counselling under BCECEB follows Bihar's own reservation policy, fee structure, and choice-filling window, which can differ meaningfully from what a candidate from another state might be used to.
In clinical practice and in mentoring residents through their own postgraduate journeys, one pattern shows up almost every year: candidates rank their choices based on the "prestige" of a college's name rather than checking what clinical exposure and patient volume that specific department actually offers. A quieter college with strong bedside teaching in medicine or surgery can serve a resident better than a bigger name with an overcrowded department.
This is worth pausing on. Bihar produces postgraduates across most major specialities β general medicine, general surgery, gynaecology, paediatrics, orthopaedics, dermatology, and increasingly, subspecialities like gastroenterology and endocrinology are getting more visibility in the state's referral hospitals.
If you're weighing branches, it helps to look at what kind of patient load and career options each speciality opens up locally. Bihar already has a working base of specialists across cities β you can browse practising gynaecologists, orthopaedic specialists, general surgeons, paediatricians, and gastroenterologists to get a sense of how these specialities are practiced once training ends. Even browsing dentists or laparoscopy surgeons near your target city can tell you how saturated or open a field is.
Patna, being the state's medical hub, gives a fair picture of demand β you can check how many general physicians, dermatologists, gastroenterologists, ophthalmologists, and dieticians are already practising there, and cross-reference that against where new PG seats are opening.
Once a seat is allotted, reporting deadlines are strict β miss the window and the seat typically goes back into the pool. Carry your allotment letter along with the documents listed earlier, and confirm the exact reporting venue with the college directly; venues have occasionally changed at short notice in past cycles.
If you're relocating to a new city for your residency, it's worth getting familiar with local healthcare access before you're buried in ward duty. Knowing where to find doctors near you or the nearest hospital isn't just useful for patients you'll treat β it helps when you or a family member needs care and you don't yet know the city.
Counselling schedules, seat matrices, and cut-offs are revised by BCECEB and MCC almost every cycle. Don't rely on last year's PDF circulating in a WhatsApp group. Cross-check dates on the official BCECEB and MCC websites before you plan travel or take leave from your internship posting.
