Learn the key preeclampsia symptoms, warning signs after 20 weeks, and when pregnancy-related high blood pressure needs urgent medical care.

Preeclampsia symptoms can be easy to dismiss, which is exactly why the condition deserves attention. A pregnant woman may have high blood pressure without feeling unwell, while another may develop a severe headache, vision changes or upper abdominal pain that needs urgent assessment.
Preeclampsia usually develops after 20 weeks of pregnancy and involves high blood pressure with signs that another organ system may be affected. The kidneys, liver, brain, blood cells and lungs can be involved. It can also develop after childbirth, known as postpartum preeclampsia.
Doctar's [guide to preeclampsia and neurological warning signs] Clonus and Preeclampsia: A Critical Guide for Expectant Mothers covers several warning symptoms, including headache, vision problems, upper abdominal pain and shortness of breath. (doctar.in)
High blood pressure is the central clinical sign, but symptoms can vary. Some women have no noticeable symptoms at all, which makes regular prenatal blood-pressure checks particularly important.
Possible warning signs include a severe or persistent headache, blurred vision or flashing lights, pain under the ribs or in the upper abdomen, nausea or vomiting that is new or severe, sudden swelling of the face or hands, and difficulty breathing.
Doctar's [preeclampsia and chronic hypertension guide] Chronic Hypertension with Superimposed Preeclampsia: A Guide for Expectant Mothers describes these symptoms alongside changes in blood pressure, urine protein and organ-function testing. (doctar.in)
A headache that is unusually severe, persistent or different from your normal headaches deserves medical attention during pregnancy, particularly when it occurs with high blood pressure.
Do not assume every pregnancy headache is preeclampsia. Headaches have many causes. The point is that a severe or persistent headache during pregnancy should be assessed rather than simply ignored.
Changes in vision can include blurred vision, seeing spots or flashing lights, or temporary loss of vision. These symptoms can reflect involvement of the nervous system and need prompt medical evaluation.
Doctar's [eclampsia symptoms guide] Understanding Eclampsia: Exploring Its Types and Impact on Pregnancy discusses severe headache and visual disturbances as important warning signs on the preeclampsia-eclampsia spectrum.
Pain in the upper abdomen, particularly beneath the right ribs, can occur when preeclampsia affects the liver. Some women may mistake it for indigestion or an ordinary pregnancy discomfort.
Doctar's [pregnancy shoulder-pain guide] Understanding Shoulder Pain During Pregnancy: Causes, Symptoms, and When to Seek Help also explains why upper abdominal pain associated with preeclampsia should not be confused with routine musculoskeletal discomfort.
Mild swelling of the feet and ankles is common during pregnancy. Sudden or marked swelling, especially involving the face or hands, is more concerning when it occurs with other symptoms.
Doctar's [pregnancy swelling guide] Pregnancy Swelling Got You Down? Compression Socks to the Rescue! notes that sudden swelling accompanied by headache, vision changes or upper abdominal pain warrants medical discussion.
The problem is that symptoms are not always obvious. A woman can have elevated blood pressure and developing organ involvement while feeling relatively normal.
In clinical practice, this is often missed because patients understandably judge their health by how they feel. Preeclampsia does not always give an early warning that feels dramatic. A blood-pressure reading at a prenatal appointment can reveal something the patient cannot feel.
That is why skipping routine antenatal visits can be risky, particularly later in pregnancy.
Doctar's [pregnancy care guide] Navigating Your Pregnancy: A Trimester-by-Trimester Guide for Indian Expectant Mothers explains the role of regular prenatal monitoring and screening during different stages of pregnancy.
Doctors do not diagnose preeclampsia from symptoms alone. They consider blood pressure measurements, urine testing, blood tests, symptoms and the baby's condition.
Urine may be checked for protein, which can indicate kidney involvement. Blood tests can assess platelet levels and liver and kidney function when clinically indicated.
Doctar's [eclampsia management article] Eclampsia: A Serious Pregnancy Complication and Its Management describes blood-pressure monitoring, urine testing and blood tests as part of assessment for severe pregnancy-related hypertension.
An ultrasound or other fetal monitoring may also be recommended when doctors need to assess fetal growth or well-being.
Doctar's [20-week ultrasound guide] The 20-Week Ultrasound: Your Guide to the Anatomy Scan explains the purpose of the anatomy scan and the role of ultrasound in pregnancy assessment.
Preeclampsia and eclampsia are not the same condition. Eclampsia refers to seizures occurring in the setting of preeclampsia and is a medical emergency.
A seizure during pregnancy or after delivery requires immediate emergency care. Do not wait for a scheduled appointment or try to determine the cause at home.
Doctar's [eclampsia overview] Eclampsia: Understanding Seizures During Pregnancy explains the relationship between preeclampsia and eclampsia and describes seizures as a critical escalation.
Another Doctar resource, [understanding different types of eclampsia] Understanding Eclampsia: Exploring Its Types and Impact on Pregnancy, discusses seizures occurring before, during or after delivery.
Delivery does not always end the risk immediately. Postpartum preeclampsia can develop after a baby is born, including in women who did not have obvious preeclampsia during pregnancy.
Severe headache, vision changes, upper abdominal pain, breathing difficulty, sudden swelling or a seizure after delivery should be treated as warning signs requiring urgent medical assessment.
Doctar's [postpartum eclampsia information] Understanding Eclampsia: Exploring Its Types and Impact on Pregnancy explains that eclampsia can occur during pregnancy, labour or the postpartum period.
Some pregnancies carry a higher risk of preeclampsia. Previous preeclampsia, chronic hypertension, kidney disease, diabetes, autoimmune disease, multiple pregnancy and certain other maternal or pregnancy factors can influence risk.
Risk does not mean a woman will develop the condition. It means her obstetric team may recommend closer observation or preventive measures based on her individual history.
Doctar's [high-risk pregnancy care guide] Best Obstetrician in Chatra: Finding Expert Maternity Care discusses high-risk pregnancy monitoring, including blood pressure, gestational diabetes and fetal growth.
Women with chronic hypertension need particular attention because preeclampsia can develop on top of existing high blood pressure. Doctar's [chronic hypertension resource] Chronic Hypertension with Superimposed Preeclampsia: A Guide for Expectant Mothers covers this situation in detail.
A gynecologist-obstetrician can assess symptoms and determine whether further investigation is needed. Complex cases may require maternal-fetal medicine or other specialist input.

Do not wait for the next prenatal appointment if you develop a severe or persistent headache, new vision changes, severe upper abdominal pain, significant difficulty breathing, a seizure or another sudden serious symptom.
A marked change in the baby's usual movements also deserves prompt assessment.
If you are pregnant or have recently delivered and something feels seriously wrong, contact your maternity team or seek emergency medical care. Preeclampsia can become serious quickly, and symptoms cannot reliably tell you how severe the underlying problem is.
