Testicular pain is one of those symptoms men tend to delay acting on β sometimes out of embarrassment, sometimes because they assume it'll pass. But the causes vary enormously, from a simple pulled groin muscle to testicular torsion, which can cost a man his testicle if not treated within hours.

Testicular pain is not something to sleep on β literally or figuratively. Most men know something feels wrong the moment it happens, but far too many wait days, sometimes weeks, before speaking to a doctor. That delay can matter enormously, depending on what's behind the pain.
Pain in or around the testicles (the male reproductive glands that sit inside the scrotum) can come from the testicle itself, from surrounding structures, or sometimes from a completely different part of the body. In clinical practice, this last point is the one most often missed β pain that originates in the kidney or lower back can travel downward and feel identical to testicular pain. It's called referred pain, and it catches patients and sometimes doctors off guard.
Here is a grounded look at the main causes.
Testicular torsion happens when the spermatic cord β the cord that carries blood to the testicle β twists on itself and cuts off blood supply. It typically causes sudden, severe pain, often with nausea and swelling. The testicle can be lost within four to six hours if blood flow isn't restored.
This is a surgical emergency. If you or someone you know develops sudden, intense scrotal pain, go to the nearest emergency service immediately. Do not wait to see if it improves.
Torsion is most common in teenage boys and young men, though it can happen at any age. It's one of the few urological conditions where the clock genuinely starts the moment symptoms begin. and urological surgeons need to act fast.
The epididymis is a coiled tube behind each testicle that stores and transports sperm. When it becomes inflamed β usually from a bacterial infection β the result is epididymitis. The pain tends to build gradually over a day or two, unlike the sudden onset of torsion.
Sexually transmitted infections like chlamydia and gonorrhea are the most common culprits in men under 35. In older men, urinary tract bacteria are more often to blame. Either way, it's treatable with antibiotics, but ignoring it can lead to chronic pain or fertility problems.

If the testicular ache comes with back or flank pain, nausea, or blood in the urine, kidney stones should jump to the top of the differential. A diagnostic centre with imaging facilities can confirm it quickly.
Testicular cancer is relatively rare but is the most common cancer in men between the ages of 15 and 35. It doesn't always cause pain β many men notice a painless lump or swelling first. When pain does occur, it's often a dull ache or feeling of heaviness.
The good news is that testicular cancer has high survival rates when caught early. Men should get into the habit of self-examination. Any unexplained lump, swelling, or persistent ache should be reviewed by a urologist without delay.

Post-vasectomy pain is a recognised syndrome where men experience chronic scrotal discomfort after the procedure. It's not discussed often enough, and men who develop it should speak to a urologist rather than assume it's normal.
Sudden severe pain: go to emergency care right now β torsion must be ruled out.
Pain with fever, discharge, or burning urine: see a general physician or urologist the same day.
A lump or swelling that doesn't hurt: still see a doctor promptly β painless doesn't mean harmless.
Pain that persists for more than a few days: book a urology consultation.
In boys and teenagers, testicular torsion is the primary concern with sudden scrotal pain. Adolescents are actually at higher risk than adult men. Parents should not take a "let's wait and see" approach with a child or teenager who complains of sudden scrotal pain. Take them to a pediatric emergency or the nearest hospital immediately.
